Northern Light Home Care and Hospice in South
Portland, Maine, is seeking a per-diem, compassionate, and skilled
nurse practitioner or physician associate to join our dedicated hospice care
team.
In this rewarding role, you will review hospice eligibility,
assess patient charts, and provide clinical guidance to nursing staff who
perform community-based home visits. While this position is primarily
remote, there may be occasional opportunities for face-to-face
visits.
