Nurse Practitioner (NP) - Family Medicine Position Available

The Family Nurse Practitioner (FNP) provides primary care to a diverse population of homeless and low-income children and adults, in a Federally Qualified Health Center. The FNP provides equitable, team-based integrated and culturally respectful patient- centered care. The FNP diagnoses, treats and manages acute and chronic health conditions, and provides preventive care focused on health risk factor reduction and patient self-management.
