The Family Nurse Practitioner (FNP) provides primary care to a diverse population of homeless and low-income children and adults, in a Federally Qualified Health Center. The FNP provides equitable, team-based, integrated, and culturally respectful patient-centered care. The FNP diagnoses, treats and manages acute and chronic health conditions, and provides preventive care focused on health risk factor reduction and patient self-management. The FNP works in a care team environment.
Outside In is an FQHC in downtown Portland, providing primary care to a diverse population of homeless and low-income children and adults and other under-served populations.
