• Practice Location
o West Union Family Health Center
• Office Hours/Work Schedule
o Office Hours: 8:00a-8:00p
o (6 or 7) 12 hour shifts per two week pay period; Rotating weekends
o Lab, x-ray and pharmacy have same hours of operation as the UC services
o Family practice, CT, MRI and Ultrasound are by scheduled appointments
• Staffing
o 1 nurse per provider, 2 providers per day
o 4 Advanced Practitioners
• Case Load
