POSITION OVERVIEW
The Social Worker/Behavioral Health Coordinator provides resource navigation and limited therapy services as a member of a multidisciplinary primary care team. Under the supervision of an LISW, provides person and family-centered, culturally-competent evidenced based treatments through face-to-face therapy with individuals and or families including, but not limited to: diagnostic assessment, treatment of mental, emotional, and substance use disorders.
