Community Action Partnerships of Western Nebraska (CAPWN) is a non-profit community-based health and human services organization that serves low-income, disadvantaged and those unable to meet their needs through other sources. The agency was established in 1965 and provides services to over 9,000 individuals, children and families in three main areas:
Community Health Services: WIC, Commodity Supplemental Food, Foster Grandparents, Migrant Seasonal Head Start, and Child Development Center.
Clinical Health Services: Primary Care, Reproductive Health, Diabetes Management, Immunizations, Migrant Health, Ryan White HIV; Behavioral Health including Counseling Services, Medication Management, Medication Assisted Treatment and Substance Use Counseling and Intensive Outpatient Services and Dental Services.
Supportive Health Services: Family Stabilization, a Teen Outreach Program, SOAR, Youth Leadership Program, Youth Shelter and Housing Assistance Programs.

Clinical Health Services Director
Exempt | Monday-Friday
Area of Responsibility: Provide overall leadership and oversight for Health Center by managing activities, planning, and adequately staffing the programs in the department. Ensure compliance by implementing programs in accordance with agency policies and procedures, grants, contract guidelines, and mandated regulations.
Major Duties:
Promote, oversee, and play an active role in delivering value-based patient care using the provider-led, team-based Patient Centered Medical Home Care Model.
Collaborate in the provision of care and services that are “accessible, continuous, comprehensive, evidence-based, family-centered, coordinated, compassionate, and culturally effective.”
Facilitates and guides the day-to-day operations of the Health Center programs activities and development of procedures, protocols, policies, and staffing patterns to carry out organizational plans and goals.
Play an active role in the development of long and short-term goals and objectives for services, facilities and staff engagement that align with the agency’s strategic plan and regulatory guidelines.
Supervise, guide and evaluate staff per organizational structure. Provides oversight of health information systems, quality assurance, and customer service processes in the clinic. Provides oversight of Migrant Health and Ryan White Part C clinical programs. Oversight of nursing staff throughout the clinic.
Performs supervisory administrative tasks including approving time off requests and timecards, setting goals, conducting annual reviews and developing performance improvement plans or corrective action as needed, conducting candidate interviews and defining successful onboarding processes.
Encourage team management, collaboration, problem-solving skills and positive feedback from staff, leaders and departments.
Communicate the agency’s mission, management decisions and agency initiatives to Health Clinic staff to enhance coordination within the agency.
Work with Human Resources staff in recruiting and hiring adequate staff for Health Clinic departments.
Develop and/or plan staff training that ensures program compliance and enhances professional growth.
Develop policy and procedures to ensure compliance with funders and regulatory requirements ensuring staff are trained and accountable to safe procedures as defined by policy.
Provide oversight and direction for the Quality Assurance and Quality Improvement processes for the Health Center.
Assist in developing Health Center department budget recommendations and resources to support the department activities including providing input in the writing of grant applications in coordination with the CEO; responsible for monitoring expenses and accurate coding of invoices in the finance process.
Address patient/client concerns or grievances following the policy with appropriate leaders and ensure communication of outcomes to appropriate stakeholders to ensure improvement.
Maintain a healthy, collaborative team environment, striving to openly communicate with staff, peers, and patients, breaking down communication barriers when necessary.
Promotes good public relations with patients, visitors, and the community.
Engage in community partnerships and collaborative efforts that increase and maintain program visibility.
Be cognizant of community needs and assist in developing programs as guided by the mission and role of CAPWN.
Ensures practices that provide a safe environment for patients and staff.
Attends required meetings including as assigned including Senior Staff, monthly Board Meeting and Board’s Health Committee.
Work with Grounds and Facilities Manager to ensure grounds and building are safe and serviceable.
Facilitates and coordinates meetings as needed.
Maintain confidentiality of records and communications following HIPAA privacy and security policies and procedures.

Working Conditions:
Clinic setting including training, meetings, and community interaction.
Requires ability to meet tight timelines.
Work in a fast-paced program with many interruptions and interactions.
May require out-of-town travel.
Physical Requirements:
Ability to bend, stoop, swat, crouch, push and pull file drawers.
Fine manipulation of hands for repetitive action.
Operate office machines.
Must regularly talk, hear and have close vision ability.
Occasionally lifting and carrying up to 25 pounds.
Minimum Qualifications:
Grade 12:
Bachelor’s degree in Nursing, Health Administration, Public Administration, or related area, preferred.
Four years supervisory experience required or two years’ experience in managing a healthcare facility or department preferred.
Age 21 or above.
Valid driver’s license and good driving record and insured vehicle.
Experience with diverse cultures and educational backgrounds preferred.
Grade 13:
Meet the minimum qualifications above;
Four years’ experience in managing a health care facility, public health program, or related programs of similar focus and budget.
Grade 14:
- Meet the minimum qualifications above;
Six years’ experience in managing a health care facility/department, public health program, or related programs of similar focus and budget OR
Specialized training/education applicable to position.
Grade 15:
Meet the minimum qualifications above;
Master’s degree in Nursing, Health Administration, Public Administration, or related area; with two years supervisory experience; and two years’ experience in managing a healthcare facility/ department, non-profit, or related entity required.
