Community Action Partnerships of Western Nebraska (CAPWN) is a non-profit community-based health and human services organization that serves low-income, disadvantaged and those unable to meet their needs through other sources. The agency was established in 1965 and provides services to over 9,000 individuals, children and families in three main areas:
Community Health Services: WIC, Commodity Supplemental Food, Foster Grandparents, Migrant Seasonal Head Start, and Child Development Center.
Clinical Health Services: Primary Care, Reproductive Health, Diabetes Management, Immunizations, Migrant Health, Ryan White HIV; Behavioral Health including Counseling Services, Medication Management, Medication Assisted Treatment and Substance Use Counseling and Intensive Outpatient Services and Dental Services.
Supportive Health Services: Family Stabilization, a Teen Outreach Program, SOAR, Youth Leadership Program, Youth Shelter and Housing Assistance Programs.

Immunization Coordinator - LPN
Full-time - Monday-Friday, 8AM-5PM
Areas of Responsibility: Dual role includes responsibility for overseeing and coordinating the Adult and Child Immunization Program and providing direct patient care within the LPN scope of practice. Duties include vaccine inventory management, compliance with state and federal immunization guidelines, public outreach, data reporting, collaborating with nurse and provider teams to support clinic operations, patient education, and health maintenance activities.
Major Duties:
- Play a vital role using the provider led, team-based Patient-Centered Medical Home Care Model. This Clinic Setting model represents the provision of care that is delivered in an efficient, effective, and courteous manner that is “accessible, continuous, comprehensive, evidence-based, family-centered, coordinated, compassionate, and culturally effective.
- Function as a Licensed Practical Nurse (LPN) and integral team member in the provision of health care with a commitment to proving excellent patient care and ability to work independently and as part of a team in a dynamic and fast-paced environment.
- Oversee, coordinate, and implement program activities in the Immunization Department.
- Follow all Nebraska Vaccines for Adults (VFA) Program and Vaccines for Children (VFC) Program requirements, policies, and procedures; update Immunization Program policies and procedures as needed to maintain compliance with State and Federal guidelines.
- Develop and oversee VFC budget and workplan, provide ongoing financial oversight, implement workplan objectives, and complete quarterly/annual reporting requirements.
- Act as the clinic’s Immunization Champion by staying current with the ever-changing immunization schedules, guidelines, formulations, and special considerations, and providing immunization education, guidance, and support to staff and patients as needed.
- Coordinate employee vaccination efforts per program policies and procedures, including annual staff flu clinic and new hire vaccine requirements.
- Work within the LPN’s scope of practice to perform the skills listed below:
- Patient assessment and monitoring, including preventative screenings.
- Prepare patients for exams and explain procedure(s).
- Administer medication as ordered.
- Administer immunizations.
- Assist with minor procedures.
- Wound care and dressing changes.
- Data collection and reporting.
- Routine lab and sample collection (including phlebotomy).
- Triage and assist in emergency situations as needed, according to the provider’s written, verbal, or electronic orders.
- Provide emotional support and counseling to patients, caregivers, and families as needed.
- Assist with the external referral process, assist with medication refills, and respond to patient inquiries with timely follow-up.
- Act as a patient advocate and practice effective patient communication.
- Provide health education to patients and families about their conditions, medications, at-home selfcare plans and health promotion activities.
- Work cooperatively with all staff and programs, external agencies, and with all program staff to meet agency/program goals; become knowledgeable of the community and agency resources.
- Ensure clinical areas and equipment are clean and sterile.
- Ensure compliance with healthcare regulations and standards.
- Practice cultural competence with the culturally diverse population this clinic serves.
- Effectively communicate with patients, families, and staff, recognizing the complexity in language interpretation.
- Maintain a positive and professional attitude, striving to openly communicate with co-workers and patients, breaking down communication barriers when necessary.
- Maintain confidentiality of records and communications following CAPWN and HIPAA privacy and security policies and procedures.
- Complete all documentation thoroughly and in a timely manner to ensure patient records are up to date and accurate.
- Attend staff meetings, out of town trainings, community outreach including satellite vaccination clinics, and workshops or classes, as requested. These activities may require irregular working hours.
- Maintain current Basic Life Support (BLS) certification.
- Maintain continuing education requirements.

Working Conditions:
- Well-lit, temperature-controlled clinical setting, with standard medical and office equipment available; noise level is above average.
- May work irregular hours, some evenings, occasional off-site vaccination/outreach clinics, and occasional out-of-town trainings.
- Work in a program in a state of change.
- Work often requires meeting tight deadlines.
- Work in a highly dynamic clinic where patients can be difficult and there are frequent interruptions throughout the day.
Physical Requirements
Ability to frequently bend, stoop, twist, stand and walk.
Manual dexterity sufficient to operate office and medical equipment.
Must regularly talk, hear and have close vision ability to record and communicate appropriate documents and records.
Occasionally lifting and carrying up to 25 pounds.
Physically able to reposition patients and lift arms above shoulder level.
Minimum Qualifications:
Grade: 14
- Current LPN license for the state of Nebraska.
- Current BLS certification.
- Strong interpersonal skills and proficient level of oral and written communication
- Promote teamwork and ability to motivate staff.
- Advanced level of organizational and multi-tasking abilities.
Grade: 15
Meet the minimum qualifications above;
Three years’ LPN nursing experience preferably in clinical nursing, women’s health, public health, or community health nursing.

Six years’ LPN nursing experience preferably in clinical nursing, women’s health, public health, or community health nursing.
