Community Action Partnerships of Western Nebraska (CAPWN) is a non-profit community-based health and human services organization that serves low-income, disadvantaged and those unable to meet their needs through other sources. The agency was established in 1965 and provides services to over 9,000 individuals, children and families in three main areas:
Community Health Services: WIC, Commodity Supplemental Food, Foster Grandparents, Migrant Seasonal Head Start, and Child Development Center.
Clinical Health Services: Primary Care, Reproductive Health, Diabetes Management, Immunizations, Migrant Health, Ryan White HIV; Behavioral Health including Counseling Services, Medication Management, Medication Assisted Treatment and Substance Use Counseling and Intensive Outpatient Services and Dental Services.
Supportive Health Services: Family Stabilization, a Teen Outreach Program, SOAR, Youth Leadership Program, Youth Shelter and Housing Assistance Programs.

Infection Control & Immunization Program Coordinator
Exempt- Monday-Friday, 8AM-5PM
Areas of Responsibility: Manage the adult and child immunization program. Coordinate surveillance, disease investigation and epidemiology services; patient case management; quality assurance related to BPHC, OSHA, and NE HHS; function in the RN role in the provision of health programs, health screening and test procedures providing client assessment, care, education.
Major Duties:
Play an active role in patient care using the provider-led, team-based Patient Centered Medical Home Care Model.
Utilize the Electronic Medical Record (EMR) and the Nebraska State Immunization Information System (NESIIS) to deliver quality care, track patient trends, and maintain immunization documentation.
Follow all Nebraska Immunization Program, Adult Immunization Program, and Vaccines for Children (VFC) Program requirements, policies and procedures. Update CAPWN Immunization Program Policies and Procedures, as needed, to comply with State and Federal guidelines.
Assist with annual and quarter reporting requirements, immunization and PHEP grant management and writing.
Provide immunization education to staff and coordinate employee vaccination efforts per program policies and procedures, including annual staff flu clinics.
Work under direction of the Nebraska Department of Health and Human Services (NHHS) and the county health department provide epidemiology services for the CAPWN Health Center. Provide assistance anywhere in the state, as requested by NHHS, in the event of a communicable outbreak and/or bioterrorism threat.
Assist Nurse Manager with participation in monthly DHHS surveillance calls and meetings relevant to epidemiology and infection control prevention.
Report communicable diseases and other reportable conditions as directed by State of Nebraska and follow proper protocols placed by state and this facility. Monitor trends in targeted communicable and chronic disease incidence.
Maintain disease specific protocols for targeted diseases and events and bioterrorism events.
Facilitate implementation of and ensure compliance with infection control procedures, OSHA, and blood-borne pathogen regulations.
Ensure and organize adequate safety and disease reporting training for new personnel and facilitate safety and disease reporting continuing education for existing clinic staff.
Oversee CAPWN’s Emergency Preparedness activities and represent CAPWN as a core member of PRMRS, our local healthcare coalition.
In collaboration with CAPWN’s Emergency Preparedness Team (EPT) and Safety Committee, maintain a complete and current Emergency Operation Plan (EOP) that satisfies national emergency preparedness requirements as set forth by the Centers for Medicare and Medicaid (CMS) Emergency Preparedness Rule.
Act as clinic safety officer. Responsible for providing a healthy workplace, assuring the facility is free of hazards and in compliance with all federal, state and local regulations including participation in agency safety committee.
Work within the RN’s scope of practice. Function as an integral team member, assist in the provision of care, helping put patients at the center of the health care system, and assist with providing primary care that is “accessible, continuous, comprehensive, evidence-based, family-centered, coordinated, compassionate, and culturally effective.”
Act as patient advocate and practice effective patient communication; assist in case management, patient population management, and test tracking as well as supporting patient care by coordinating services with both community resources and other Community Action Partnership of Western Nebraska (CAPWN) program areas.
Maintain a positive and professional attitude, striving to openly communicate with co-workers and patients, breaking down communication barriers when necessary.
Attend staff meetings, out of town trainings, community outreach including satellite vaccination clinics, and workshops or classes, as requested. These activities may require irregular working hours.
Maintain confidentiality of records and communications following CAPWN policy and HIPAA privacy and security policies and procedures
Maintain current Basic Life Support (BLS).

Clinic environment, training, meetings, and community interaction.
Well-lit, temperature-controlled clinical setting, with standard medical and office equipment available.
Work is frequently stressful and requires meeting tight deadlines.
Work in a highly volatile program in a state of change with many interruptions and interactions.
Must be able to work irregular hours, some weekends and evenings and out of town training.
The noise level in the work environment is above average.
Physical Requirements
Ability to frequently bend, stoop, twist, stand and walk.
Manual dexterity sufficient to operate office and medical equipment.
Must regularly talk, hear and have close vision ability to record and communicate appropriate documents and records.
Occasionally lifting and carrying up to 25 pounds.
Physically able to reposition patients and lift arms above shoulder level.
Minimum Qualifications:
Grade: 7
Current Nebraska license as an RN with a minimum of one year’s experience preferably in clinical nursing, public health, or community health nursing.
- Possess knowledge and skills in preventive care and childhood illness.
- Current BLS certification.
Valid Nebraska driver’s license with good driving record and insured vehicle.
- Proficient oral and written communication, teamwork and ability to motivate staff, advanced level of organizational skills and function with minimal supervision.
- Proficient knowledge in Word, Excel, Outlook, etc.
- Requires ability to select and interpret appropriate guidelines or procedures, which may be vague and/or complex

Meet the minimum qualifications above;
RN with three years of nursing experience preferably in clinical nursing, public health, or community health nursing.
Grade: 9
Meet the minimum qualifications above;
Six years of nursing experience preferably in clinical nursing, public health, or community health nursing.
- Satisfactory completed the required Applied Epidemiology Course.
- Specialized training in related field(s).
Grade 10
- Meet the minimum qualifications above;
- Ten years of nursing experience preferably in clinical nursing, public health, or community health nursing.
