We are looking for a mission driven Referral Care Coordinator to join our team!

Referral Care Coordinators assist patients through full lifecycle medical visits by scheduling appointments and follow-up services within and outside the clinic as directed by providers. Serves as a liaison between the clinic, patient and community service providers to ensure patient receives quality and timely care.

This is a full-time position located on our main campus (30th & L). The schedule will be 40 hours per week; Monday-Friday, 8am to 5pm.

QUALIFICATIONS:

High school diploma and/or equivalent and basic computer skills required.

Associate’s Degree in Health Administration and/or a related field preferred.

Medical office and/or customer service experience preferred; 1-2 years of experience preferred.

The ability communicate effectively with internal and external customers.

Demonstrated ability to: analyze and solve problems; prioritize and organize; exhibit a high level of integrity and sense of urgency to service the customer.

Displays initiative as a team player and as a leader.

Proficiency in English and Spanish required.
